Community Nursing

Under the Community Nursing service our qualified support workers offer basic medical support within a community setting. It covers essential nursing and disability services that are key to the overall well-being of the NDIS participants. At LinkAssist, we understand that each participant will have unique requirements and that’s why we first conduct assessments to best understand the needs of the participants and offer personalised and comprehensive nursing assistance.

Our community nurses have the experience, knowledge and qualifications required to offer basic medical assistance. They work within the guidelines of the NDIS, prioritise participant well-being and offer quality support.

As part of our Community Nursing support service, our support workers will offer assistance with –

  • Managing your medications
  • Pressure care and wound management
  • Diabetes management
  • Bowel care
  • Continence management
  • PEG and ventilator management
  • Complex care
  • Developing overall support and care plans
  • Palliative care

At LinkAssist, we constantly strive to accommodate and deliver to the changing needs of the NDIS participants. Based on your prescriptions and required medical care, we will improve our service delivery and ensure all-around assistance.
